Cytoreductive Surgery (CRS), often referred to as "debulking" surgery, is a sophisticated, intensive surgical procedure designed to remove all visible tumors from the abdominal cavity. It is primarily used to treat cancers that have spread to the lining of the abdomen (peritoneum), such as appendiceal cancer, colorectal cancer, or ovarian cancer.

Cytoreductive Surgery

The goal of CRS is "complete macroscopic resection," meaning the removal of all tumors large enough to be seen. In many instances, this is combined with HIPEC—Hyperthermic Intraperitoneal Chemotherapy—where heated chemotherapy drugs are circulated through the abdomen immediately after the tumor removal to destroy any remaining microscopic cancer cells.

The Procedure

CRS is a complex intervention that may involve the removal of several abdominal organs, including portions of the colon, the gallbladder, the spleen, or parts of the stomach, to ensure all tumor deposits are cleared. Because this is a high-stakes procedure, it is performed in specialized centers by surgeons with advanced training in peritoneal oncology. The meticulous nature of this surgery is what allows it to offer patients a chance at long-term disease control in scenarios where other treatment options may be limited.

Who Is a Candidate?

Not every patient with abdominal cancer spread is a candidate for CRS. Eligibility is determined through a rigorous assessment process, including detailed cross-sectional imaging (like CT or PET/CT scans) and, often, a diagnostic laparoscopy to map the extent of the disease. Factors such as the patient's overall fitness, the biological behavior of the cancer, and the potential for achieving a complete resection are critical. Recovery and Support

Recovery from CRS is a journey that requires time, patience, and dedicated rehabilitation. Patients typically spend time in the Intensive Care Unit (ICU) immediately following surgery, followed by a hospital stay focused on nutritional stabilization and physical recovery.
